Recognize a loved one through a memorial or honorary gift. Include the name of the person you are recognizing and the contact information for the individuals who you would like to be notified about the gift. Make a tribute gift.

The easiest way to create a planned gift is by including a bequest in your will. If you wish to make a will bequest, please use the following language:

“I give and bequeath [written amount or percentage of my estate or description of personal property] to Northwest Hospital Foundation, EIN # 25-1483624, a nonprofit corporation currently located at 100 Fairfield Drive, Seneca, PA 16346, for its general charitable uses and purposes.”

You may also name Northwest Hospital Foundation as a beneficiary of your retirement account or life insurance policy. The Foundation would receive the full amount, tax-free. Conversely, if your IRA or life insurance is left to family members, the distribution is subject to taxes. Contact us at 814-676-7145 for more information.

Donating appreciated stock or assets, making an IRA charitable distribution, or giving from a Donor Advised Fund are great ways to make a difference while maximizing your tax benefits. Consult your tax advisor and contact us at 814-676-7145 before initiating your gift.

You can honor the memory of a loved one who has passed away by mentioning Northwest Hospital Foundation in their obituary.

For memorials specific to UPMC Northwest Cancer Center:

“Memorial contributions may be made to Northwest Hospital Foundation, 100 Fairfield Drive, Seneca, PA 16346, to support the Hillman Cancer Center at UPMC Northwest, or online at NorthwestHospitalFoundation.org.”
